Jen will file insurance claims at the usual and customary rate to your insurance company for you. Insurance fees paid by your insurance plan are based on my contracted rates with your insurance or EAP to provide in-network services. You will owe what your plan dictates for your co-pay, applicable co-insurance, and/or deductibles. You may owe the usual and customary rate, a lesser rate or only a co-pay depending on your insurance plan. Please call your insurance carrier to find out the details of your particular plan to determine your out of pocket costs.
Payment is expected at the time of service for self-pay. If you have insurance, I will file the claim and charge the authorized card on file for any deductible or copay owed when the claim is finalized, unless other arrangements are made.
I accept cash, check, credit/debit, and health savings accounts (HSA). I require a card on file for co-payments, deductibles, no show, and late cancellation fees that insurance will not pay for. I use Stripe via SimplePractice for card payments transactions.
If you do not have insurance or you choose to not utilize your insurance and pay out-of-pocket, you will be provided with a good faith estimate and insurance opt-out form for anticipated services.
Please understand that your hour is reserved specifically for you at a specific time and that the hour cannot be filled by another client without at least 24 hours of notice. Please provide me the courtesy by canceling or rescheduling your appointment at least 24 hours in advance. If you are ill, please do not come into the office. If you find out you’re ill same day as appointment, I’m happy to offer a telehealth option instead for that appointment in lieu of a cancelation fee.

What services will my plan cover?
Services may be covered in full or in part by your health insurance or employee assistance plan. Please check your coverage carefully by asking the following questions:
SELF-PAY:
You will be considered Self-Pay if you do not have insurance, if you present without valid/updated insurance information, if you opt-out of insurance, or if the services provided are not covered by your insurance. All Self-pay services must be paid in full on the date of service unless other arrangements are made. If your insurance denies coverage or does not cover services you will be responsible for the amount owed in its entirety. I require a credit/debit card on file. You may also pay for services with your HSA card.
COUPLES THERAPY
Couples therapy is out of pocket only, unless specifically covered by your EAP like Lyra, or other insurance plans, check with your insurance for benefits information.
Insurance generally does not pay for couples treatment.
Couples therapy is $150 per hour.
EAP:
Ask your employer about mental health EAP benefits, as I may be able to accept your EAP payment for therapy services, including for couples therapy.
Due to the NO SURPRISES ACT, as of January 1, 2022, You have the right to receive a “Good Faith Estimate” explaining how much your services will cost. Prior to your first scheduled appointment, you will receive a good faith estimate of the services you are seeking via email or my client portal.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You are entitled to receive this “Good Faith Estimate” of what the charges could be for psychotherapy services provided to you. While it is not possible for a psychotherapist to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you. This estimate is not a contract and does not obligate you to obtain any services from the provider listed, nor does it include any services rendered to you that are not identified here.
This Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time.
